4/25/11 Community Health Status Assessment meeting minutes

1. Community Health Status Assessment Objectives
         • To determine the health status of the community
         • To gather data for important health indicators
2. Activity timeline (over next 3-5 months)
         • Review existing CHSA data (Core Indicators)  SE CHSA

            CHSA Core Indicator Lists

                   ii. Socioeconomic Characteristics – Ali

3. Identify data indicators that are particular interest to the community (4/25/11)
            • Homeless event data/Front st clinic data
            • Housing availability
            • Transportation
            • Play areas/trail/physical activity resources
            • % contraceptive use among teen population
4. Collect data by 6/20/11 – Send data to Masayo
5. Organize and analyze data (Next meeting 6/27/11 at 2pm at United Way)
6. Compile results into a community health profile (TBD)

Discussed MAPP phase three – 4 Assessments:

·    Community themes and strengths assessment  - Next meeting 5/17/2011 at 2pm United Way of Southeast Alaska
    Provides a deep understanding of the issues that residents feel are important by answering the questions: "What is important to our community?" "How is quality of life perceived in our community?" and "What assets do we have that can be used to improve community health?"

o   Homeless coalition event findings
o   United Way COMPAS assessment
o   Best Beginnings community assessment
o   After school coalition middle school survey
o   Capital City Forum/Juneau focus day
o   JEDC report
o   Etc.

·    Local Public Health System Assessment – possible in late Summer Focuses on all of the organizations and entities that contribute to the public's health. The LPHSA answers the questions: "What are the components, activities, competencies, and capacities of our local public health system?" and "How are the Essential Services being provided to our community?"

     Identifies priority community health and quality of life issues. Questions answered include: "How healthy are our residents?" and "What does the health status of our community look like?"

  • Find data for local specific indicators
  • Find current data if needed

·    Forces of Change Assessment – meet after other 3 assessments are done

focuses on identifying forces such as legislation, technology, and other    impending changes that affect the context in which the community and its public health system operate. This answers the questions: "What is occurring or might occur that affects the health of our community or the local public health system?" and "What specific threats or opportunities are generated by these occurrences?"
