4/25/11 Community Health Status Assessment meeting minutes
AGENDA ITEMS
1. Community Health Status Assessment Objectives
• To determine the health status of the community
• To gather data for important health indicators
2. Activity timeline (over next 3-5 months)
• Review existing CHSA data (Core Indicators) SE CHSA
– Person responsible reviewing and collecting data by 6/20/11 (Send data to Masayo)
CHSA Core Indicator Lists
i. Demographic Characteristics – Sean -----2000 & 2010 population
ii. Socioeconomic Characteristics – Ali
iii. Health Resource Availability – George
iv. Quality of Life – Masayo
v. Behavioral Risk Factors – Natasha
vi. Environmental Health - Natasha
vii. Social and Mental Health – Lisa
viii. Maternal and Child Health – Ashley
ix. Death, Illness, and Injury – Ellen D
x. Communicable Disease – Ellen M
xi. Sentinel Events – Yvonne
3. Identify data indicators that are particular interest to the community (4/25/11)
• Homeless event data/Front st clinic data
• Housing availability
• Transportation
• Play areas/trail/physical activity resources
• % contraceptive use among teen population
4. Collect data by 6/20/11 – Send data to Masayo
5. Organize and analyze data (Next meeting 6/27/11 at 2pm at United Way)
6. Compile results into a community health profile (TBD)

7. Disseminate community health profile - web, presentation, paper, etc.
