·         Some questions to ask:
o    75% of the 2.7 trillion dollars we spend on healthcare in this country is spent on treating diseases that are largely preventable, such as heart disease and diabetes. Why do you think this is happening? What can be done to change this statistic?
o   According to the film, why are primary care physicians so important?
o   Roy Litten is a patient in the ER who has had several heart attacks and a stent placed in his heart, but continues to eat eggs and bacon and to smoke. He has stopped taking his medicine because it is too expensive, and he doesn’t have his own doctor; when he started having chest pains he had to resort to going to the ER. What could be done by each of the stakeholders in the healthcare system to help a person like Roy start on the road towards good health? What are the challenges and obstacles in the way?
o   What do you think of workplace financial health incentive programs like Safeway’s? Should private and public insurance implement similar “behavior as currency” strategies?

Next Juneau MAPP Local Public Health System Assesment Subcommittee Meeting

Dear community partners,
Our next Juneau MAPP local public health system assessment (LPHSA) subcommittee meeting is on 3/22/12 from 1:30pm to 3:30pm at the Juneau Public Health Center 1F conference room.
We will be answering the following two essential public health service questionnaires.
Agenda
1:35pm-2:30pm: #1 – Monitor Health Status to Identify Community Health Problems
o   Accurate, periodic assessment of the community’s health status
o   Utilization of appropriate methods and technology, such as geographic information systems, to interpret and communicate data to diverse audiences
o   Collaboration among all local public health system components, including private provides and health benefit plans, to establish and use population health registries, such as disease or immunization registries
2:35pm-3:30pm: #2 – Diagnose and Investigate Health Problems and Health Hazards in the community
o   Epidemiological investigations of disease outbreaks and patterns of infectious and chronic disease and injuries, environmental hazards, and other health threats.
o   Active infectious disease epidemiology programs
o   Access to a public health laboratory capable of conducting rapid screening and high volume testing

Invitation to Juneau MAPP Local Public Health System Assessment Orientation meeting 3/1 at 1:30pm

Dear Friends and Community Partners:
We’re pleased to invite you to attend the inaugural kick-off meeting of the Juneau MAPP (Mobilizing for Action through Planning and Partnerships) Local Public Health System Assessment Subcommittee.
This local public health system assessment is one of four Juneau MAPP (Mobilizing for Action through Planning and Partnerships) community health assessments and is integral in gathering necessary information for our next phase identifying strategic issues in our community. 
A public health system is made up of all public, private and voluntary entities that contribute to the delivery of essential public health services within a jurisdiction. On March 9, 2011, a group of fifty-plus community members met, brainstormed, and collectively gave life to their vision for a healthier community:  “Juneau, a thriving community – engaging in a diverse and intergenerational population and ensuring access to an exceptional quality of life.” 
In order to measure the performance of our local public health system, it is important to have those representatives from organizations and sectors involved in our local public health system participate.  The CDC will analyze our completed assessment, and provide feedback- in a report that can be easily used to identify strengths and weaknesses of the public health system in Juneau.
As a Juneau resident, being a member of the Juneau MAPP group provides you with a unique opportunity.  A person with your agency’s experience and insight would be the perfect partner in spreading the word with people, just like you, who want to help improve the quality of our health and life by contributing their input a vision of a healthier Juneau.
Our orientation and first assessment meeting will be conducted in the Egan Room of Centennial Hall on Thursday, March 1, 2012. The full process will run from 1:30pm to 4pm.
We will begin promptly at 1:30pm with registration, followed by a brief orientation at 1:45pm, with the actual assessment process at 2:30pm; a packet containing the assessment tool will handed out we will discuss and complete it together, and finish at or before 4:00pm.
Subsequent meetings to complete the rest of the assessment tool will be held on Thursday, March 22, 2012, Thursday, April 5, 2011, and Thursday, April 19, 2012 from 1:30pm to 3:30pm at the Juneau Public Health Center. 
We forward to having you join together with other community partners in taking a close look at our public health system.

9/27/11 Community Themes and Strengths Assessment Subcommittee Meeting Minutes

Identified Themes and strengths:
  • Volunteerism
  • Cultural and recreational opportunities
  • Strong sense of community
  • Nature/environment
  • Quality educational infrastructure;  including optional programs
  • Health and social services available
  • Diversity of jobs available
  • Affordable energy (hydropower)
  • Large percentage of population votes (statistic was first cited as 87% of population, then a 66% citation found
 Identified Needs:
  • Quality Childcare
  • Affordable housing
  • Better Mental Health Services
  • Healthy relationships
  • Youth activities
  • Access to health care
  • Transportation –(still some discussion/uncertainty whether this is an identified need)
  • Employment opportunities/equal pay for equal work
  • Substance abuse prevention and treatment

9/12/11 Juneau MAPP Community Health Status Assessment Subcommittee meeting minute

On 9/12/11, Juneau MAPP CHSA subcommittee met at the United Way of Southeast Alaska. We reviewed the draft 2011 Juneau Community Health Profile. We will be meeting again on 10/24/11 2-3pm at the Juneau Public Health Center to talk about 10 community health indicators to monitor over the next 3-5 years and ways to present the data to our community.

Community Themes and Strengths Assessment Subcommittee meeting 8/16/11

We had a great MAPP CTSA meeting on 8/16 – 18 people attended!

Here is a quick summary:

In this assessment, we want to know:

·         What is important to our community?
  • How is quality of life perceived in our community?
  • What assets do we have that can be used to improve community health?

These are themes and strengths identified after reviewing existing assessment/review of information for special groups:
·         Coalitions/Groups/activities
·         Facilities/Infrastructure (fitness, arts, etc.)
·         Transportation
·         Nature/environment (including trails, etc.)
·         Educational infrastructure
·         Access to health care
·         Affordable housing
·         Better mental health services
·         More jobs/higher wage (seafood industries, fishing, tourism, government, mining, health care, etc.)
·         Affordable energy (hydropower)
·         Quality & quantity child care
·         Violence prevention & education
·         Healthy relationship
·         Substance abuse prevention and treatment
·         Youth activities
·         Respecting diversity
 Next step:
·         Is this enough information we need to answer the questions?
o   On 9/27/11, we will be looking closely at the 2005 COMPASS assessment themes and strengths
·         Nee three levels of information gathering –
o   Open discussion to elicit community concerns, opinions, and comments in an unstructured way. Asking open-ended questions ensures that issues of concern and interest to the community are raised.
o   Perceptions regarding community quality of life. Questions about quality of life in the community can identify specific concerns. Information on perceptions about quality of life in a community can be gathered through a community survey/or community discussions.
o   A map of community assets. Asset mapping is an important tool for mobilizing community resources. It is the process by which the capacities of individuals, civic associations, and local institutions are inventoried.  (Nursing student, Eddie Locci to create a asset map of Juneau)
·         Compile the results of the community themes and strengths assessment and share it with the entire community.
o   How? (We will be discussing this on 9/27/11)
·         Summarize key findings (two to five pages) for guiding work during the Indentify Strategic Issues phase. (in future)

Community Health Status Assessment Subcommittee meeting minutes 6/27/11

We reviewed findings and synopsis for each category:

  1. Demographic Characteristics  – aging population, more diversity, report shared
  2. Socioeconomic Characteristics  - pending report
  3. Health Resource Availability – divide between people with access and without access, there are a lot of people who don’t get to medical care, report shared
  4. Quality of Life – report shared
  5. Behavioral Risk Factors  – report shared
  6. Environmental Health  - report shared
  7. Social and Mental Health - report shared
  8. Maternal and Child Health   - some data was not local, pending report
  9. Death, Illness, and Injury  – leading cause of death, injury, cancer, heart disease, pending report
  10. Communicable Disease  – was not able attend, pending report
  11. Sentinel Events - pending report
We discussed about next steps:
  • Combine all data into a health profile
  • Review the health profile & make comments
  • Disseminate the health profile and findings for the community
  • Discuss indicators to monitor in future and identify challenges
We also talked about Juneau Commission on Sustainability and their sustainability indicators. This might be good information for Juneau MAPP to review.
